Insurance Coverage

We do accept most major insurances at this time including BCBS, UHC, Cigna, Ambetter and Aetna. We strive to make healthcare accessible and affordable for all our patients. If you don't have insurance, we do offer out of pocket visits at an affordable price. Look below for our payment plan options.

Payment Options

We offer a multitude of different payment options. We accept all major credit cards, HSA and FSA, and offer multiple payment plans through affirm, scratch pay, and cherry. Payment plans are easy to do and apply to all services. You can apply for a payment plan through scratch pay below!

Rooted Health Hormones & MedSpa – SMS/Text Message Terms & conditions 

By opting in to receive SMS (text) messages from Rooted Health Hormones & MedSpa (“we,” “our,” “us”), you agree to the following terms and conditions:

1. Program Description

 

Our SMS program may send you:

 

  • Appointment reminders and confirmations

  • Limited, non-sensitive treatment follow-up messages

  • Special offers, event invitations, and promotions (if you opt in)

  • Wellness tips and service updates

 

 

We will never send protected health information (PHI) via SMS unless you have provided explicit written consent.

 

2. Opt-In & Consent

By providing your mobile phone number and affirmatively opting in (through a written form, web form, or text confirmation), you consent to receive recurring automated and/or manual text messages from Rooted Health Hormones & MedSpa.

 

  • Consent is not a condition of receiving services or making purchases.

  • Message frequency will vary but is typically 1–4 messages per month.

3. Opt-Out

You may opt out at any time by replying STOP to any message.

  • After opting out, you will receive one final message confirming your unsubscribe request.

  • You may rejoin at any time by providing your consent again.

5. Message & Data Rates

Message and data rates may apply from your mobile carrier.

You are responsible for any charges from your carrier for SMS messages received from us.

6. Carriers

Mobile carriers are not liable for delayed or undelivered messages.

Delivery is subject to effective transmission from your carrier and your device’s SMS capabilities.

7. Privacy & Security

Your privacy is important to us. We will only use your mobile number as described in our Privacy Policy.

SMS communications are not encrypted; therefore, we will not send sensitive medical details through text without your explicit written authorization in compliance with HIPAA.

8. Arkansas-Specific Notice

In accordance with the Arkansas Personal Information Protection Act, you will be notified promptly if there is a security breach involving your mobile number or other personal data collected for SMS purposes.

9. Changes to Terms

We reserve the right to modify these SMS Terms & Conditions at any time. Updates will be posted on our website with the “Effective Date” updated accordingly. Continued participation in the SMS program after changes are posted constitutes acceptance of the updated terms.
